Evidence of Insurability
If you elect supplemental coverage for yourself within 60 days of the date you become eligible to participate, Evidence of Insurability is not required for coverage levels up to six times your salary under $1.2 million. Elections made after the first 60 days of eligibility and elections of over six times salary or $1.2 million and more require Evidence of Insurability.
Your spouse or domestic partner does not have to provide Evidence of Insurability if you elect a coverage level at or below $30,000 and you enroll within 60 days of hire.
Your spouse or domestic partner must provide Evidence of Insurability if you elect a level of coverage over $30,000 and/or you do not enroll within 60 days of your hire. If you have a life event resulting in a qualified family status change, you may make changes to your elections or enroll for the first time within 31 days of the life event change. If due to a qualifying family status change, spouse coverage of one increment of $10,000 is available as Guaranteed Issue coverage if your application is received by MetLife within 31 days of the qualifying family status change. You do not need to provide Evidence of Insurability for your eligible child.
