  • Megavitamin and nutrition based therapy
  • Nutritional counseling for either individuals or groups, including weight loss programs, health clubs and spa programs, health clubs and spa programs except when it is either:
    • a medically necessary preventive service for children and adults who are obese, and for adults who are overweight and have other cardiovascular disease risk factors, when it is prescribed by a physician and furnished by a provider recognized under the plan (e.g. a licensed nutritionist, registered dietician, or other qualified licensed health professionals such as nurses who are trained in nutrition); or
    • medically necessary in treating chronic disease states (e.g. diabetes mellitus, eating disorders, gastro-intestinal disorders, hypertension, kidney disease, seizures), and other conditions in which dietary adjustment has a therapeutic role, when it is prescribed by a physician and furnished by a provider recognized under the plan (e.g., a licensed nutritionist, registered dietician, or other qualified licensed health professionals such as nurses who are trained in nutrition).
  • Nutritional and electrolyte supplements, including infant formula, donor breast milk, nutritional supplements, dietary supplements, diets for weight control or treatment of obesity (including liquid diets or food), food of any kind (diabetic, low fat, cholesterol), oral vitamins, and oral minerals except when sole source of nutrition or except when a certain nutritional formula treats a specific inborn error of metabolism.
