When Coverage Starts and Ends
You are covered under this program on your first day of employment or the date you become eligible.
Coverage ends on the date when the first of the following occurs:
  • • you no longer meet the eligibility requirements
  • • you terminate employment
  • • your death
  • • the program is terminated.
